I think your epoches are set too high, causing this to be "overbaked". 10-15 epoches is normally best (figuring out when within that range is a matter of looking at the preview images and seeing when stuff you didn't train but used in the prompts for the preview images, like background details, starts decaying. If you can't tell, 10-12 are generally safe from overbaking if your dataset isn't tiny, though 13-15 will be better if they don't experience overbaking). If you still have one of the older epochs in that range I'd recommend adding it as a version.

"New" version (TEST) works a lot better. Definitely less overbaked. From here I think the main issue is you didn't tag her whole outfit, and didn't have a proper trigger word for the character.
